Important Note for FSSAI State License Renewal:

• FBO's who want to apply for fssai license renewal must apply for fssai lience renewal at lease 1 week prior to the license expiry date to avoid very high late renewal penalty charges i.e 3 times or 5 times of Fssai License Govt Challan Amount i.e post expiry date.
• 3 Times late renewal penalty applied after expirty and within 90 days of fssai license expiry date
• 5 times late renewal penalty will be applied after 90 days and within 180 days of fssai license expiry date

• Blueprint/layout plan of the processing unit showing the dimensions in metres / square metres and operation-wise area allocation List of Directors / Partners / Proprietor / Executive Members of Society / Trust with full address and contact details with nomination of authorized signatory

• Name and List of Equipments and Machinery along with the number, installed capacity and horsepower used

• Analysis report(Chemical & Bacteriological)of water to be used as ingredient in food from a recognized/ public health laboratory to confirm the portability indicating the name of authorized representative of Lab who collected the sample and date of collecting sample. (Not Applicable if water is not being used as ingredient)

• Photo I.D and address proof issued by Government authority of Proprietor / Partner / Director(s) / Authorised Signatory.

• Proof of possession of premises. (Sale deed/ (Own Property) / Rent Agreement ) with latest Electricity bill, / Water Bill etc.,

• Partnership Deed / Self Declaration for Proprietorship / Memorandum & Articles of Association (AOA) towards the constitution of the firm / Copy of certificate obtained under Coop Act-1861/ Multi State Coop Act-2002 in case of Cooperative
